Eight words. People always ask me what we do at Physician's Way, and it really comes down to 8 words. "We help people lose weight and get healthy." That is really the major goal at our clinic. We accomplish this by 1) Helping patients reign in their daily intake of calories in a way that satisfies their hunger, as well as their nutritional needs. 2) We strongly encourage the development of ones fitness, while simultaneously helping patients learn how to feed their bodies in a way that supports optimal fitness. 3) We give unconditional moral support and provide accountability with weekly weigh-ins. 4) We assist this process with medications which help with appetite control, in those individuals who need help with this aspect, and in whom there are no contraindications to do so.
That's what we do. "We Help People Lose Weight and Get Healthy."

Have a Plan for the Holidays!

If your don't already know how your going to approach the holiday season, it is not too late to get your plan in place....better late than never!
For starters, plan to get your journal out and pay close attention to your food choices by writing everything down that passes your lips. Also, think hard about how you know that you'll feel if you over-indulge at any time over the next 6 weeks. Think about where you want to be, how you want to feel, and how much you want to weigh on January 1st 2008.
It is possible to buck the trends that so many people expect, and actually continue to lose weight over the holidays.
Focus on higher protein (eat the turkey), low carb (limit the pies, cakes, candies, mashed potatoes and stuffing), low fat ( limit the giblet gravy, butter, and sauces). Also, limit your alcohol intake, if you drink.
Make it a point to exercise at least a little everyday over the holidays.
Give yourself a big holiday gift this year....A healthier you at the beginning of the pending New Year!

What Type Foods Should I Eat in the Evening?

A lot of patients ask me what types of foods they should be eating after 6pm. In reality, the answer lies in how much exercise you are doing each day and how well controlled one's calorie intake is during the early part of the day (weight-loss is a calories 'In' versus calories 'Out' phenomena). In general, when the main goal of a patient is weight-loss, it can be very useful to decrease complex carbohydrate (white rice, bread, pasta, potatoes, torillas...etc.) after 6pm. Complex carbohydrate is very efficiently converted to glycogen in the liver for fuel storage, with the remainder stored as bodyfat if it is not utilized for the fuel demands of daily living or burned through exercise. I typically encourage patients to eat 'Lean and Green' during the evening, focusing on eating lean protein (fish, chicken breast, tofu, turkey, lean beef) and getting their carbohydrates through green and colorful vegetables without adding unhealthy dietary fats for flavor. As we typically 'wind down' during the evening and demands for fuel energy decrease as we prepare for sleep, eating 'Lean and Green' is a strategic approach to helping your body utilize bodyfat for fuel while you sleep. For the final meal of the day, an essentially pure protein meal such as low-fat cottage cheese, grilled chicken breast or turkey slices may help your body elevate it's metabolic rate while you rest. Protein requires your body to work a little harder to utilize, as it is essential for the repair of muscle cells and other body components, and is not so readily converted to fat for storage, as would be the case with a high fat/high carb meal before bed.

A Bitter Pill to Swallow

In the practice of Bariatric (weight-loss) Medicine, we commonly employ the use of prescription appetite suppressants to help patients with appetite control issues. These medications are often useful and helpful, specifically for patients who have trouble controlling the number of calories that they consume on a regular basis, and as a jumpstart to aid the momentum required to begin a dietary program.

Occasionally, however, I am faced with a patient who just wants a 'pill' and expects that this will take care of their weight problems. Unfortunately, there simply is no magic bullet. There is no magic pill or medication which will remove the unwanted and unhealthy pounds which are the result of too many calories and too little activity.

Successful, long-term weight-loss is almost always the result of a conscious effort on one's part to monitor their calorie intake, and make regular exercise a way of life. To live a long, active, healthy life, many of us will at one point in our lives, be in the position of having to take a close look at how we regularly eat, and whether or not we make exercise a priority. This assessment may be the result of undesired weight gain and dissatisfaction with how we look, or our doctor informing us that we have developed hypertension, type 2 diabetes, or some other potentially weight related illness.

Regardless of the reason you may have for needing to lose weight, please understand that you must make a lifestyle change, if you want long-term weight control and health. Rx appetite suppressants can help in the process, but it is not the end-all, rather, they are simply a tool in the overall process. You must make the decision that you are willing to change your life, and live it in a healthier manner.
